MADELEY GREEN DAY - 1st October 2006

Please return this form to Madeley Parish Council, Jubilee House, 74 High Street, Madeley,
by Thursday 28th September
1. Madeley Green Festival is open to the public from 1.00 to 4.00.
2. Vehicular access is from Station Road, via the underpass, as shown on the enclosed map. Vehicles should be off the site by 12.45 and not return until after 4.00pm.
3. Your stall position is .........as shown on the map. Numbered pegs will be placed at the appropriate locations on the day.
4. We ask you to provide your own tables chairs and a gazebo or shelter. If you are unable to supply these items please contact Madeley Parish Council as soon as possible.
5. All stalls or activities must have adequate adult supervision.
6. Please wear appropriate clothing and footwear, conditions underfoot may be wet.
7. There are two toilets on sites.
8. Please take care at the end of the day and make sure that all litter and personal belongings are removed from the site.
9. Help can be obtained from officials who will be wearing high visibility jackets and vests.
